Occupational Therapy

1) A patient must be home-bound and a physician must order Home Health Care Services.

Home-bound is defined as –

  • leaving the home is not recommended due to your condition
  • you cannot leave home without assistance such as a wheelchair, special transport, walker or receiving help from another person
  • leaving home is taxing and requires considerable effort

2) Services must be specific, safe, and effective for your condition

3) Your condition is complex and services can only be effectively performed by a qualified therapist

4) One of the following exists:

  • It is anticipated that your condition will improve in a predictable amount of time
  • A skilled therapist is necessary to safely and effectively establish a maintenance program for your condition
  • A skilled therapist is needed to safely and effectively perform maintenance therapy
  • The amount and duration of service is reasonable.

The Occupational Therapist assigned to your care and the other professionals from our interdisciplinary team will monitor your progress and consult with your physician in order to help him/her decide how often and how long the visits will be. If you have Medicare coverage, the home health care ordered by your doctor is usually covered. If you are covered under a Medicare Advantage plan, you may be subject to a deductible or co-payment equal to a percentage of the total cost of care. The agency will verify the home health care benefits covered under your health plan prior to the start of care, and will inform you of any out-of- pocket expense you may be responsible for.
